More than 75,000 DVDs resulting from CSB investigations have been distributed to industry and labor groups, government agencies, safety trainers, educators, emergency responders, and individual requesters throughout the world, the agency said.
"Our big take-home message is, wear self-contained breathing apparatus during all phases of firefighting," said co-author Suzanne Snedeker. "Put on the gear before arriving at a fire and don't take it off until the operation or inspection is completed, even during fire inspections conducted days later when chemicals can still outgas from charred remains."
Seven repeat offenses for similar fall hazard violations in 2006 and 2007 alone resulted in $100,000 in proposed penalties.

The ongoing investigation of the accident by OSHA is a good start, but that the CSB, which investigated the 2005 explosion at a BP refinery in Texas City, is especially suited to look into this kind of industrial accident, says U.S. Rep. Gene Green.
Changes include a revised point system, changes in credit by degree level and field, changes in credit for other qualifications, and a new minimum experience requirement.
The CSB's final report concluded the root causes of the accident were that the refinery did not have an effective program to identify and freeze-protect piping and equipment that was out of service or infrequently used; that the refinery did not apply the company's policies on emergency isolation valves to control fires; and that current industry and company standards do not recommend sufficient fireproofing of structural steel against jet fires.
The incorrectly wired equipment was sold at RadioShack stores nationwide from October 2004 through January 2008 for between $50 and $85.
The one-hour session will include a 30- to 40-minute presentation on ionization, photoelectric, and dual technology smoke alarms, along with an ask-the-experts segment.
